I understand that camp fees are non-refundable and I will not be given a refund upon cancellation,
unless a child is placed in my absence.
I understand that in case of emergency, Camp Northbrook will make every reasonable effort to
contact me and the physician named on this form. If this is not possible, Camp Northbrook has my
permission to call 911, and I am responsible for payments of any medical charges or expenses not
covered by my insurance or the insurance applicable to my child (if any).
I understand that if my child is ill, I will not send them to camp and I will not receive a refund.
I understand that my child will not be released into custody of any person who is not authorized by
appearing on this list.
